Maybe the tan is simply because the Otohime family is leading a very traditional lifestyle. Mutsumi's father is probably a fisherman, and they cultivate small fields as well. Those who are too young to help are taken along to play outdoors. (I wonder if the idyll we see has anything to do with modern countryside reality in Japan.)
I think Mutsumi is an exceptional child among her siblings. Her anemia has, to some extent, prevented her from taking part in the physical work and other outdoor activities. Instead she has had time to concentrate on school more than anyone else, so much so that she eventually became a Tokyo University student. My conclusion is that if Mutsumi visited her family towards the end of winter (as much as there is "winter" on Okinawa), the differences between skin colors would be barely noticeable.

Is the Motoko-sama look alike your talking about the black haired girl closest to the reader with the sword-case looking object over her shoulder about an inch (an inch on the page) next to the motorcycle? Quote:
