released on the Mega Drive in 1989, Altered Beast (never one of my favourites) was a side-scrolling beat-em-up. The player must battle armies of fictional and undead creatures on his way through the levels, at the end of each is the evil Demon God Neff in various forms, who is holding Athena captive, and your objective is his defeat and Athena's rescue. Along the way, the player has to obtain power-ups which increase his strength and size. These enable the player to turn into three stages of superhuman. When three are collected, the hero transforms into the titular beast with exceptional powerful abilities, but a beast change isn't needed to complete the game (which you just lost, by the way).

Yea, it's the grand pappy of most brawler games. Very simple but at the same time was designed pretty well.
